Appointments and Fees

Step 1: Get in touch
Intake appointments can be scheduled by contacting our office. During your first call, you can expect some general questions about yourself (or your child) and your/their needs. Please ask your questions, too! Our intake coordinator will then help you schedule a virtual intake visit.

Step 2: Intake
You’ll receive an email with a link for your virtual intake appointment. The email will also have online forms (e.g., registration and cost estimate, background history, privacy policies). You may submit other information such as previous evaluations and school records. During the virtual intake, you will participate in a thorough interview and develop a working evaluation plan designed around your specific needs. At the end of your intake session, you’ll schedule in-person testing.

Step 3: In-person Assessment
Testing typically takes place over one or two 1/2-day sessions at our Fairfield office. Bring a snack and arrive well-rested. You (and your child) are in good hands!

Step 4: Feedback Session
Within 2- to 3- weeks of testing, Dr. Crystal will contact you to schedule a virtual feedback meeting. This capstone meeting includes an interactive review of the evaluation report and detailed recommendations.

FAQ

  • General estimates are provided by phone when you call our office. More detailed estimates are given in the Registration form received prior to the virtual intake visit. Invoices are provided and due at the time of in-person testing at the office. Cash, check, and all major credit cards are accepted.

  • Our office is currently able to submit claims to Medicare only. Other clients are self-pay. For those who wish to use insurance, our office will provide you with a superbill (statement of services, charges, and billing codes) that you can submit directly to your insurance company for reimbursement. If you plan to do so, be sure to call member services to find out about your out-of-network benefits and rates of reimbursement.

    By not working directly with insurance companies, our office is able to ensure a higher level of privacy and center treatment decisions around your needs. Insurance companies often limit types/amounts of services and require specific diagnoses for coverage.

  • Dr. Crystal’s office requests a 24-hour notice for cancellations. If you do not appear for a scheduled appointment or cancel the appointment with less than 24 hours notice, a missed appointment fee will be charged.

  • Yes! Dr. Crystal’s office offers telehealth options. Please contact our office for more information.

  • Yes, Dr. Crystal does IEEs! Please be in touch to get started with this process.
